Job Description:
A company is looking for a RN Ambulatory Case Manager - Remote
PST.
Key Responsibilities:
Prioritize patient care needs and address emerging issues during
initial visits
Coordinate care and communicate with patients, families, and
healthcare teams regarding treatment plans
Monitor high-risk patients and ensure continuity of care through
effective discharge planning
Required Qualifications:
Graduation from an accredited school of nursing
Active, unrestricted Registered Nurse license through the State of
California
1+ years of experience in a clinical setting or case management
