Healthcare Social Service Manager: Plan and manage activities of in a community- based day care center to provide social services to aged adults and adults with chronic medical, cognitive or behavior health conditions

- review medical and financial conditions; assess needs for therapy, counseling, nursing, healthcare, social and living programs; conduct psychosocial evaluation with GDS and MMSE to develop IPC; provide individual counseling; develop healthcare options; develop discharge plans; implement compliance policies.

Req.: Master of Social Work from a US accredited university; 2 yrs' prior on-job work exp.

in social work/service for adult care facility; possess certificate of training or course completion in TF-CBT, Motivational Interviewing and Cultural Competency; $70,990/yr.
