Key Responsibilities
Provide telephonic patient care and support for medication
adherence and disease state education
Collaborate with healthcare professionals to develop and evaluate
comprehensive care plans
Utilize telecommunications nursing outreach programs to ensure
continuity of care for defined patient populations
Required Qualifications
Registered Nurse with an unrestricted current compact license in
Texas
Ability to be licensed in multiple states
3+ years of clinical RN experience
Experience using Microsoft Office, including Word, Excel, and
Outlook
Candidate must reside in Texas
