Job Description:
A company is looking for an RN Care Coordinator to work
remotely, providing patient-first healthcare and managing care for
high-risk patients.
Key Responsibilities
Collaborate with a multidisciplinary care team to develop goals and
plan interventions for high-risk patients
Coordinate care and disease management, including conducting
comprehensive clinical assessments
Serve as a liaison for patients and families, assisting with
transitions of care and connecting to community resources
Required Qualifications
Graduate from an accredited School of Professional Nursing
Current state licensure as a Registered Nurse (RN)
Basic Life Support (BLS) Certification through the American Heart
Association (AHA)
Three to five years of nursing experience
Preferred: Bachelor's of Science in Nursing (BSN) and specialty
certification
