A company is looking for a Care Manager (RN) OB.
Key Responsibilities
Evaluate member needs and barriers to care, recommending
personalized care plans
Develop ongoing care plans and collaborate with providers to
address unmet needs
Coordinate care and follow up on member status to ensure
appropriate services are received
Required Qualifications
Degree from an accredited School of Nursing or Bachelor's degree in
Nursing
2 - 4 years of related experience
RN - Registered Nurse - State Licensure and/or Compact State
Licensure required
