The Licensed Vocational Nurse (LVN) utilizes knowledge of basic nursing principles, practices, and ethics in order to perform procedures and tasks within their scope of practice, but for children with very special needs.The LVN is responsible for providing and documenting nursing care, under the supervision of a Registered Nurse, in accordance with developed care plan and physician's orders for each individual patient and coordinates care among all members of the interdisciplinary healthcare team.It is expected that the employee will demonstrate behavior consistent with the Core Values. The employee shall support the organization strategic plan, the goals and direction of the quality assurance performance improvement process, and activities.SPECIFIC FUNCTIONAL RESPONSIBILITY:Assessment and PlanningInitiates, updates, and documents age appropriate plan of care in conjunction with interdisciplinary team assessments and goals to ensure quality and continuity of care for patientsPerforms patient assessments and reassessments and collaborates with the Registered Nurse to develop appropriate plans of care, and nursing interventionsCollaborates with the Registered Nurse (RN) to actively assess pain management and implements prescribed medication and/or other alternative methods that are effective for the patientAssists in patient/caregiver teaching with appropriate documentation as assignedDemonstrates knowledge, recognizes the signs of abuse and implements professional response, reporting, and documenting according to laws and organization policiesRecognizes and responds competently to acute medical emergencies according to established procedures and scope of practiceMedication and Treatment SafetyApplies medication and treatment safety practices by using the six rights to medication and treatment administration: right patient, right medication/treatment, right route, right dose, right time, right approachAdministers medication as ordered by physician and documents patient's response to medications and treatments within scope of practiceCommunicates with pharmacies on new/refill medications, and checks medication deliveries for accuracyReceives, processes, transcribes and reconciles medication orders timely and accuratelyDemonstrates knowledge of appropriate techniques and treatment modalities as performed on patients of varying ages, including infant, toddler, adolescent and young adultsDemonstrates clinical competency in nursing care to technically dependent patients (such as tracheostomies, gastrostomies, etc.)Recognizes abnormal laboratory values/diagnostic reports and notifies the Registered Nurse in charge and/or physiciaDocumentation and Reporting:Measures and documents patients vital information and responds appropriatelyObserves and documents patients signs and symptoms of illness and therapeutic treatments in medical recordDocuments and reports changes in patient condition/unusual events utilizing approved reporting systemCompletes all documentation timelyProvides accurate hand-off communication using SBAR reporting to ensure patient safety and continuity of carePatient Care Area-GeneralPlans, organizes, and prioritizes daily work using sound judgment and following the direction of the Charge Nurse, Nurse Manager, and physicianEngages patients with a positive and friendly demeanor, while providing patient and family centered careEnsures and follows infection control policies and current practicesParticipates in training and orientation of new personnel as assignedOversees patient care provided by the Certified Nursing Assistant (CNA) during assigned shiftMission Statement: Mountain View Child Care, Inc. is dedicated to enriching the health and quality of life of children with medical and developmental needs and who benefit from habilitation and rehabilitation services and the supportive use of technology.
